تقديم طلب التسجيل
يُقدم طلب التسجيل لأغراض الضريبة الانتقائية إلى الوزارة على النموذج المعد لهذا الغرض، على أن يتضمن الطلب على الأقل البيانات والمعلومات الآتية:
1- البيانات الشخصية لمقدم الطلب.
2- الشكل القانوني للمنشأة التي يزاول من خلالها نشاطه.
3- شهادة القيد بالسجل التجاري بالمملكة.
4- شهادة القيد بالسجل الصناعي عند مباشرة نشاط الإنتاج.
5- نوعية السلع الانتقائية التي تتعلق بها الأنشطة التي يرغب في التسجيل عنها.
What does the Article mean in plain language?
Application data establishes applicant identity, legal form, commercial registration, industrial registration for production, and goods. Match names, identifiers, activities and evidence and distinguish the person from individual premises. An incomplete or inconsistent form may require supplementation and does not itself establish registration.

How can this rule appear in practice?
If invoices name one company and the application names its sister company, reconcile the applicant and evidence before submitting.
